Heather Deel, APRN, FNP-C
PRIMARY CARE PROVIDER
Heather graduated from Indiana Wesleyan University with her family nurse practitioner (FNP). She is currently providing family medicine and primary care services at our clinic. Heather is available to see patients for acute sick visits and chronic care management, including diabetes, hypertension, COPD, addiction, etc.
Georgette Greene, APRN, FNP-C, CARN-AP, PMHNP-BC
primary care/mental health provider
Georgette graduated from Walden University with her family nurse practitioner (FNP) and Eastern Kentucky University with psychiatric mental health certification (PMHNP). She is currently providing both family medicine and mental health services at our clinic.
Dana Conn, MSW, LCSW
Dana is a Licensed Clinical Social Worker providing outpatient counseling at our clinic. Dana completed her undergraduate degree in Social Work at Morehead State University and obtained her Master's Degree in Social Work at Campbellsville University. Dana uses a blend of therapeutic approaches to support individuals in a strengths-based way. Dana has experience working with anxiety, depression, substance use disorders, trauma, and other mental health diagnosis.

Karen Smith, BSW & Ashlee Haddix, BSW
Our Case Managers are available to help clients connect to community resources, supports, and gain more independence through their recovery with mental health or substance abuse. These are valuable services that help promote the well being and long term success of our clients!

At Raven Rock, we treat even the smallest members of the family! From infants, adults, and the geriatric population. We are here for examination, diagnosis, treatment, and ongoing wellness for all age groups.
Not feeling well or need an annual exam? We do it all! We also perform school physicals, sports physicals, and CDL physicals. Whether you're sick or only need some routine maintenance, we're here for you!
Mental Health
We provide specialized mental health treatment at Raven Rock. With a psychiatric nurse practitioner (PMHNP) and licensed clinical social worker (LCSW) available, we can meet all your mental health needs, including substance abuse treatment.
Telehealth Services
Sometimes there are obstacles to coming into a medical office, but help is still available. Our providers can meet you over a HIPAA-compliant connection. Same-day appointments are available!
